The goals of PAVE Florida are design, development and sustainability of an integrated information and care coordination highway made up of networks of health care providers and entities that manage and deliver care utilizing local and regional partnerships committed to elevating quality and expanding access to those they serve. PAVE Florida is facilitating and convening partners and stakeholder organizations that seek to “PAVE a path forward”. We are reimagining healthcare as a microcosm within our communities that anticipates and addresses the complex and diverse needs of our citizens while building the infrastructure necessary to innovate new care and payment models that can be replicated in every community nationwide.
Our fundamental premise is that all citizens are entitled to equal access to medications and services that promote healthy and productive lives, address social determinants of health, integrate behavioral and physical health care and create a system that rewards continuity of care and improved outcomes.
Our mission can be accelerated and scaled through business partnerships with national, state and regional organizations that share our vision to create access and value for all by addressing barriers to access, building trust in the healthcare system and creating optimum value for the services provided.
Partner funds will be used to support activities needed for strategic, fiscal, and operational management including design and implementation of the program infrastructure, measures, reimbursement models, training, work groups/advisory councils, analytics, reporting, data exchange process, interventional components, and outcome evaluations.
